PARTICIPANT CONSENT AND WAIVER FORM

Location: Jack Byrne Arena, Torbay, Newfoundland (7 Kennedy's Brook Dr, Torbay NL A1K 1K8)

Minimum Fundraising Amount: $200
 
PARTICIPANT ACKNOWLEDGMENT

I wish to participate in the RBC Skate for the Kids hockey tournament (the "Event"). I understand that:

Participation is Voluntary: My participation in this Event is entirely voluntary. You must be 18 years or older to participate in this event (on ice).


Assumption of Risk: I acknowledge that hockey is a contact sport involving inherent risks, including but not limited to collision, falls, and injury. I understand these risks and assume all responsibility for them. I confirm that I am in good physical condition to participate and will not participate if unfit to do so.


Release of Liability: In consideration for being permitted to participate, I hereby release and waive all claims against Royal Bank of Canada, its employees, officers, directors, agents, representatives, and the venue (Jack Byrne Arena) from any liability for injury, damage, or loss arising from my participation in this Event, whether or not caused by negligence.


Compliance: I agree to abide by all tournament rules, regulations, event instructions, and applicable laws.
 
PHOTO AND VIDEO CONSENT

I give permission to Royal Bank of Canada to capture, use, and publish photographs and video recordings of me (including my name, likeness, voice, and image) for promotional purposes. This includes use on websites, social media, print materials, and other media without compensation or prior approval.
